We’ll get to know your current situation and understand your family’s vision for having the best care options possible.

We’ll draw on our lived experience and professional background in the aged care sector to identify the services available to you.

The choice will be yours. Then, we'll deal with My Aged Care for you.

We arrange home-based aged care support services for you:

Home Care Assessment and Management

  • 2-hour home visit or video appointment
  • Comprehensive assessment of physical, emotional, social and health needs
  • Written Care Plan for client, family and GP’s reference
  • Individualised advice and education
  • Complete all referrals for chosen services including to My Aged Care
  • We are the primary contact with My Aged Care for all interactions as your My Aged Care representative
  • Advocacy with My Aged Care assessors and service providers
  • Ongoing engagement following the home visit of up to 3 months to provide Care Management

 

Fee $1250 + GST

Home Care Package Advice

1-hour telehealth or phone consultation to shortlist Home Care Package (HCP) providers and ensure you understand how a Home Care Package works to get the most out of it. 

For example:

  • How many care hours you will receive
  • What you can use a Home Care Package for
  • The role of a case manager
  • Fees: explain HCP Provider fees and income-tested contributions

 

Fee $195 +GST for a 1-hour consultation 

(plus we research Home Care Package providers specific to your situation)

Care Management

  • Regular home visits to monitor your aged care situation
  • Ensuring home care services meet needs identified in the Care Plan
  • Providing carer support
  • Ongoing advice / recommendations as situations change
  • Adjusting services as needs change
  • Advocacy with My Aged Care on your behalf as needed

  

Fee $145 +GST per hour
We can arrange the support you need to remain independent and well cared for in your own home.

You’ll receive our personalised and professional support

Professional development for healthcare professionals and support workers in aged care

  • Increase knowledge about: My Aged Care, Home Care Packages, Commonwealth Home Support Program, assessment skills and more.
  • Learn about changes to aged care and home care guidelines.
  • Discuss client scenarios and clinical reasoning.
  • Maintain professional registration requirements.
