Home Care Assessment and Management

During a Holistic Aged Care Assessment, we spend 2 hours visiting a person at home to formulate a personalised plan, which includes:

A comprehensive home care needs assessment

• Understanding an older person’s physical, emotional, social and health needs
• Understanding an older person’s values and wishes
• Speaking with an older person’s important people, such as family and doctor
• Educating about health-based and home-based aged care services including My Aged Care and Home Care Packages
• Completion of a Care Plan to summarise the assessment and recommendations for the most appropriate home care services

• We stay actively involved following the initial Holistic Aged Care Assessment and home visit, providing care management for up to 3 months.
• We're the primary contact with My Aged Care for all interactions as your My Aged Care representative.
• We'll complete referrals to service providers and My Aged Care
• We're your advocate with My Aged Care, assessors and service providers
• We'll make all the arrangements to start home care services in line with an older person’s care plan.

$1250 + GST all inclusive

If you self-manage your home care package with one of the major self-managed providers below, they can cover the cost of our Holistic Aged Care Assessment.
